Inspector Notes

An unannounced monitoring inspection was conducted on 07/19/2022. There were 60 children, ages 3 months-12 years, and 16 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in the following: during movie time, in free choice play, going outside to play and during lunch. A total of 6 children?s records, 6 staff records and 6 medications were reviewed. The inspector discussed the following with staff: items from home, sanitizing, records, nutrition and medication. The inspector arrived for the inspection at 9:20 am and departed at 12:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-290-A-3
Based on observation and discussion with staff, the center failed to ensure that in areas used by children of preschool age electrical outlets shall have protective covers.

Evidence:
1. There were 5 uncovered electrical outlets on a power strip in the hallway near the restroom on the bottom floor of the center. This area of the center is used by children of preschool age.
2. There were 5 uncovered electrical outlets on a power strip near the sink in the Bears Classroom. This area of the center is used by children of preschool age.
Plan of Correction: Will cover them today.
Standard 8VAC20-780-500-A
Based on observation and discussion with staff, the center failed to ensure that children?s hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

Evidence: Child 3 was observed during a diaper change. Child 3 was observed immediately returning to activities following the diaper change. Child 3 did not wash hands with soap and running water after toileting.
Plan of Correction: Will review with the staff today.
Standard 8VAC20-780-520-A
Based on observation and discussion with staff, the center failed to ensure that all nonprescription drugs and over-the-counter skin products shall not be kept beyond the expiration date of the product.

Evidence:
1. Over-the-counter Skin Product 1, in the Piggie Classroom, had an expiration date of 05/2022.
2. Over-the-counter Skin Product 2, in the Tigers Classroom, had an expiration date of 01/2020.
Plan of Correction: Will throw them away and get new ones.
