Inspector Notes

An unannounced on-site monitoring inspection was conducted on 07/29/2024. The on-site inspection began at 9:30 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 100 children, 15 staff providing direct care and supervision, and various support staff present during the inspection. The inspector reviewed 10 children?s records and 10 staff records. The inspection included document review, a tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-160-A
Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the documentation shall have been completed within the last 30 calendar days of the date of the employment.

Evidence: The record for Staff 6 contained a documented hire date of 06/24/2024; the tuberculosis screening was dated 01/28/2024.
Plan of Correction: Will make sure that TB screenings are not older than 30 days at the time of hire.
Standard 8VAC20-780-160-C
Based on record review, the center failed to ensure that tuberculosis screenings are updated at least every two years from the date of the initial screening or testing.

Evidence:
1. The most recent tuberculosis screening in the record for Staff 8 was dated 07/22/2022; an updated screening was not obtained by 07/22/2024.
2. The most recent tuberculosis screening in the record for Staff 9 was dated 01/31/2022; an updated screening was not obtained by 01/31/2024.
3. The most recent tuberculosis screening in the record for Staff 10 was dated 05/31/2022; an updated screening was not obtained by 05/31/2024.
Plan of Correction: Will have all staff get updated screening/tests. The appointments were made during the inspection.
Standard 8VAC20-780-280-B
Based on observations, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: There was an unlocked cabinet in the Kings Kids Classroom that contained the following cleaning materials: Microban, Clorox All Purpose Spray, Great Value Disinfectant, Lysol, and Great Value All Purpose Cleaner. Children were observed in the classroom while the cabinet was unlocked.
Plan of Correction: Will get a lock on the cabinet.
Standard 8VAC20-780-530-A
Based on document review and observations, the center failed to ensure that a least one staff in each classroom or area where children are present shall have a current CPR and first aid certification.

Evidence: Staff 7 was the only staff member assigned to supervise the Toon Squad; Staff 7 does not have a current CPR and first aid certification.
Plan of Correction: A staff member with a current certification will be with the group until the staff can get the certification.
