Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 01/24/2025. The on-site inspection began at 9:15 am and ended at 1:55 pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 14 staff providing direct care. The inspector reviewed 5 children?s records and 5 staff records on-site on 01/24/2025.

The inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The additional immunizations for Child 2 (14 months old) were overdue by 6 months.
Plan of Correction: Going to make sure that the system that exists is used properly to track.
Standard 8VAC20-780-245-A
Staff shall complete a minimum of 16 hours of training annually.

Staff 2, who has been employed for more than one year, obtained less than an hour of the required 16 hours of annual training in 2024.
Plan of Correction: Will make sure that staff get the required hours of training each year.
Standard 8VAC20-780-320-B
Water temperatures from sinks should not exceed 120 degrees Fahrenheit.

The water temperature in the portable sink/diaper changing station in Classroom 5 measured 167 degrees.
Plan of Correction: Will adjust the water temperature today.
