Inspector Notes
An unannounced monitoring inspection was initiated on 01/23/2025 and completed on 02/03/2025 with a local agency in response to a self-report received by the licensing office on 01/23/2025 relating to programs. The inspector reviewed 2 children?s records and 1 staff record on-site on 01/24/2025.
The inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
The inspection findings have updated after a review by the Enforcement Consultant on 03/06/2025.
Violations
2Centers shall be in compliance with the center?s own policies and procedures that are required by the standards.
The center?s discipline policy prevents the use of abusive language and corporal punishment, including hitting, spanking, beating, shaking, pinching, and other measures that produce physical pain.
Staff 1 did not follow the center?s discipline policy on 01/23/2025 when they used corporal punishment on Infant 1 (14 months old) for biting another infant. Staff 1 struck Infant 1 in the face, resulting in the child stumbling backwards, and then pointed their finger at Infant 1 and scolded the infant for their behavior.
The following actions, or threats thereof, are forbidden, physical punishment or striking a child.
Staff 1 did not ensure that forbidden actions were not used with Infant 1 (14 months old) on 01/23/2025 when they struck Infant 1 on the face for biting another infant.
