Inspector Notes
An unannounced inspection was initiated on 02/21/2025 and completed on 02/25/2025 with a local agency in a response to a complaint received by the licensing office on 02/19/2025 relating to allegations of staffing and supervision and special care provisions and emergencies. The inspector reviewed 1 child?s record and 2 staff records on-site.
The inspection included document review, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
The inspection document was revised after a review by the licensing inspector on 03/12/2025.
Violations
6When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff 1 and Staff 2 did not provide care, protection and guidance to a group of 10 children (age two years old) in Classroom 1 on 02/17/2025. The children were observed running around the classroom, climbing on top of shelving units, jumping on cots, wrestling on cots, and throwing toys at one another. During this time, Child 1 (2 years old) sustained an injury to their forehead that required outside medical attention. Staff 1 and Staff 2 were unable to identify how the injury occurred.
The children did not wash their hands after lunch on 02/17/2025. Staff 1 changed a total of 9 children?s diapers on 02/17/2025, the children did not wash their hands after the diaper change, nor did Staff 1 wash their hands in between each of the diaper changes.
Staff 1 changed a total of 9 children?s diapers on 02/17/2025. Staff 1 did not clean or sanitize the diapering surface after each use.
The center shall maintain a written record of children?s serious and minor injuries and the record shall include the required components.
The injury record for Child 1 did not contain two signatures.
The staff in Classroom 1 did not sit with the children during lunch on 02/17/2025.
The center shall inform the department?s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Child 1 (2 years old) sustained an injury, while under the supervision of the center on 02/17/2025, that required outside medical attention. The center did not notify the department that the injury occurred.
