Inspector Notes
An unannounced monitoring inspection was initiated on 04/02/2025 and completed on 04/10/2025 in response to a self-report received by the licensing office on 04/02/2025 relating to staffing and supervision. The inspector reviewed 1 child?s record, 3 staff records, and 1 applicant record on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1On 04/01/2025, 5 children were left in the care of Applicant 1 in the lobby of the center for approximately 1 minute and 37 seconds. During this time, the children were not within the actual sight and sound supervision of center staff.
On 04/01/2025, Child 1 (5 years old) exited the center through the front door of the facility and was outside of the building for approximately 1 minute 24 seconds before being found by their parent. Child 1 was not within actual sight and sound supervision of staff during this time.
