Inspector Notes

An unannounced monitoring inspection was initiated on 04/10/2025 and completed on 04/28/2025 in response to a self-report received by the licensing office on 04/10/2025 relating to staffing and supervision. The inspector reviewed 2 children?s record and 1 staff record on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 04/10/2025, Child 1 (4 years old) was left lying on the carpet in Classroom 1 for approximately 10 minutes while the class went into the hallway, directly outside of the classroom, for a restroom break following rest time. Child 1 was not within actual sight and sound supervision of staff during this time. The center was previously cited on 04/01/2025 for leaving a child alone and unsupervised.
Plan of Correction: The staff was terminated. Will remind teachers to count and ensure all children clear the classroom.

The required ratio for children 3 years old up to school age eligibility is one staff per 10 children.

On 04/10/2025, Staff 1 was the only staff member supervising the 12 children (ages 4-5 years) in Classroom 1; two staff was required for this group of children.
Plan of Correction: Staff will be instructed to ensure that ratio is always met.

REPEAT VIOLATION

Roughly handling a child, or the threat thereof, is forbidden.

On 04/10/2025, on three separate occasions, Staff 1 roughly handled children when they used their foot to pick up one end of cots that were occupied with sleeping children. Staff 1 lifted each cot approximately 1 foot off the ground and then dropped the cot back onto the ground, jarring the children, to wake-up the sleeping children. The center was previously cited on 01/23/2025 for using a forbidden action.
Plan of Correction: Revisited proper wakeup procedures with all staff.
