Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 05/28/2025, as part of probation. The Department issued a Special Order on 04/15/2025 to place the licensee on probation for a period of six months; and to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The on-site inspection began at 10:30 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 62 children present and 17 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
The inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3In areas used by preschool age or younger children, electrical outlets shall have protective covers.
There were two uncovered electrical outlets in the hallway near the upstairs women?s restroom; the children were seated in the hallway by the uncovered outlets during a restroom break. There were two uncovered electrical outlets in the downstairs women?s restroom used by children.
Children?s hands shall be washed with soap and running water after toileting and staff shall wash hands with soap and running water before and after a diaper change.
The staff in Classroom 8 stated that the sink in the classroom was broken and that they were using hand wipes to wipe both the children?s and staff?s hands before and after diaper changes.
The children in Classroom 6 (youngest child 18 months) were served whole meatballs.
