An unannounced, on-site focused inspection was initiated and completed on 08/12/2025, as part of an Intensive Plan of Correction. The on-site inspection began at 9:15 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 93 children present and 17 staff. The inspector reviewed 10 children?s records and 9 staff records.
The inspection included document review, tour of the facility, interviews, and observations.
