Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 10/06/2025, as a part of the probationary period. The Department issued a Special Order on 04/15/2025 to place the licensee on probation for a period of six months; and the require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The on-site inspection began at 10:10 am and ended at 11:53 am. The inspector reviewed compliance in the areas listed above. There were 55 children present and 15 staff.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
