Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 9:20am and 10:30am. There were 6 classrooms observed with a total of 81 children with 13 staff within the supervision guidelines. The children were observed playing outside, practicing reciting songs, doing matching activities, eating their morning snacks and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and medications were observed during this inspection. Based on interview, the child day program does not have a carbon monoxide detector for each building that houses a child day program.
Evidence: The child day program takes place in two separate buildings. The program director confirmed that only one out of the two buildings are equipped with a carbon monoxide detector.
Plan of Correction: We can get another one installed today.
Standard 8VAC20-770-60-C-2
Based on record review and interview, a central registry background check has not been obtained for each employee within the first 30 days of employment.
Evidence:
1. Staff #1 (date of hire: 5/3/2021) record does not contain documentation of a central registry finding.
2. Staff #2 (date of hire: 8/24/2021) record does not contain documentation of a central registry finding.
3. The program director stated that she didn't know Virginia central registry background checks needed to completed for staff that were hired when they lived in a different state or are currently living in a different state.
Plan of Correction: We will get the forms in the mail today.
Standard 8VAC20-780-60-A-8
Based on interview, a written care plan has not been obtained for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: Child #5 and Child #6 have confirmed food allergies and have medications kept on site. The program director confirmed that allergy action plans have not been obtained for each child.
Plan of Correction: We will request action plans from the families to be completed within 30 days.
