Inspector Notes

An unannounced Monitoring Inspection took place on October 27 2022 between the hours of 9:51 am and 12:05 pm. There were 6 classrooms observed with a total of 52 children with 14 staff within the supervision guidelines. The children were observed participating in circle time, singing nursey rhymes, looking at books, exploring the inside of a pumpkin and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-780-160-A
Based on record review, a staff record did not contain documentation of a negative tuberculosis screening at the time of employment.
Evidence: Staff #2?s date of hire is 6/23/2022 and their TB test/screening is dated 8/20/22.
Plan of Correction: I will let admin know we must have them by the first day of employment.
Standard 8VAC20-780-440-C
Based on observations and interview, the center allowed cots, or beds, or other sleeping equipment to be stacked.
1. Rest mats are stacked together after rest time with the individual linens left on the mat.
2. The Director acknowledged the mats are stacked in classrooms with the linens on the rest mats.

Plan of Correction: I will order a heavy barrier to go in between each mat until the cabinet is built to store the mats individually.
Standard 8VAC20-780-500-B
Based on observation and interviews, the diapering surface is not being cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1. All the Infant, toddler and twos classrooms (0-35 months) do not clean the diapering surface with soap & water. Only bleach solution is being used to sanitize the diapering surface.
2. There were no bottles of soap & water available to clean the diapering surface in the classrooms (Infants, P-1, Toddlers and Twos) or (0-35 months).
Plan of Correction: I will add a soap & water bottle for the teachers to use at the diapering area and will retain my staff on proper diapering procedures.
