Inspector Notes

An unannounced monitoring inspection took place on July 10, 2023 between the hours of 9:45am and 11:00am. There were six (6) classrooms observed with a total of 50 children present with 11 staff supervising within the supervision guidelines. The children were observed playing with various manipulatives, and eating snack. A complete inspection of the physical plant, children and staff records, fire drill log, medication, required posted documents, first aid kit, and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review, the provider did not obtain documentation for a 5-year repeat.
1. Staff #3 record did not have documentation of an updated sworn statement..
2. Staff #6 record did not have documentation of an updated criminal background check.
Plan of Correction: We will get this completed as soon as possible.
Standard 8VAC20-780-130-E
Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #3 (DOB 8/5/21) last updated immunization record was 12/14/21.
Plan of Correction: I will inform the parents we need an updated immunization record.
Standard 8VAC20-780-160-C
Based on record review, staff did not submit the results of a follow-up tuberculosis (TB) test or screening every 2 years from the date of the initial screening or testing.
Evidence:
1. Staff #4 (DOH 8/15/07) most recent TB screening is dated 4/1/21 (expired 4/1/23).
2. Staff #6 (DOH 9/14/04) most recent TB screening is dated 2/26/20 (expired 2/26/22)
Plan of Correction: We will have the employee submit an updated TB screening.
