Inspector Notes

An unannounced Monitoring Inspection took place on February 6, 2024, between the hours of 9:00am and 11:20am. There were six (6) classrooms observed with a total of 46 children present with 12 staff supervising within the supervision guidelines. The children were observed playing with various manipulatives, group activities, story time, music, handwashing, eating snack, and napping. A complete inspection of the physical plant, children and staff records, evacuation drill logs, medication, required posted documents, first aid kit, outdoor play area, and emergency procedures were observed during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #3 (DOB 06/17/21) did not have additional immunizations in his record. The last immunization documentation was 10/6/22.
Plan of Correction: I will tell the parent to bring in an updated last immunization record.
