A complaint was received by the licensing office on May 1, 2024, regarding administration, staff qualifications and training, staffing and supervision, programs, special services, and special care provision and emergencies. An on-site investigation was conducted on May 2, 2024. The license inspectors conducted interviews, observations were made, and records were reviewed.
