During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/24/2026. The on-site inspection began at 10:10 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 48 children and 13 staff present. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-520-C
If diaper ointment or cream is used, these products shall be kept in an area inaccessible to children.

There were six diaper creams/ointments being stored in the cabinets under the changing station, accessible to the children (ages 1 month-14 months). There were no child-proof locks present on the cabinets being used.
Plan of Correction: The diaper creams/ointments were immediately relocated to a higher cabinet which is inaccessible to children until child-roof cabinet locks can be installed.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes the identity of the person conducting the drill, the date and time of the drill, and the method used for notification of the drill.

The documentation of emergency drills did not include the identity of the person conducting the drills or the method used for notification of the drills over the past 12 months, The date for the lockdown drill for 2025 was also incomplete.
Plan of Correction: The center has printed the model form to document future emergency drills with all required details.
