An unannounced complaint investigation was initiated with the director on June 17, 2022 beginning at 11:50 AM and ending at 1:20 PM to investigate allegations regarding supervision and staff-to-child ratio. There were 71 children, ages 6 months through 5 years old in the care of 13 staff members. The children were observed eating lunch and during their nap/resting period. Center documentation was reviewed. The allegation regarding staff-to-child ratio was determined to be valid.

Based on observation and staff interviews, the center did not ensure that there was one staff member to four children from ages zero to 16 months. Evidence: There were 10 children between the ages of six months and 2 years in the care of 2 staff.
Plan of Correction: The director stated that generally they have enough staff to maintain the correct ratio. The director stated that she and her assistant have stepped in to assist when staff-to-child ratios are not met. The director stated that she would ensure that the correct staff-to-child ratio was met in each class.

Based on observation and staff interview, the center did not comply with all of the safe sleep guidelines recommended by the American Academy of Pediatrics. Evidence: In the infant room there was a child observed in the connected classroom asleep alone in the classroom.
Plan of Correction: The director stated that she would speak with the staff in the infant room regarding putting sleeping children in a separate room. Staff will be retrained on Safe Sleep Guidelines.
