Inspector Notes

An unannounced monitoring renewal inspection was conducted on 12/05/2023 from 9:55 a.m. to 1:10 p.m. There were 98 children present, ranging in ages from four months to five years old with 25 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #3?s date of birth was 08/22/2022. The date of the last immunization on file was 05/23/2023. Staff #11 confirmed that the center had not obtain documentation of additional immunization within the last six months.
2. Child #10?s date of birth was 10/24/2022. The date of the last immunization on file was 01/13/2023. Staff #11 confirmed that the center had not obtain documentation of additional immunization within the last six months.
