Inspector Notes
An unannounced, on-site inspection was initiated on January 21, 2026 completed on January 21, 2026 in response to a complaint received by the licensing office on January 14, 2026 relating to allegations of Staff and Supervision. The inspector reviewed one staff records on-site. This inspection included document review, interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
