Inspector Notes

An unannounced inspection was initiated on and completed on 6/4/2025 in response to a complaint received by the licensing office on 5/30/2025 relating to allegations of Administration, Physical Plant and Staffing and Supervision. The inspector reviewed four children?s records and all staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
