Inspector Notes

An unannounced focused inspection was conducted on August 21, 2023 and November 1 2023 in response to a self-reported incident that occurred at the center on August 18, 2023. The center informed the licensing office that a child ingested a food allergen, was administered medication according to their medication action plan, and was transported to the hospital via emergency services. Interviews were conducted with staff, the child, and the child's guardians. A violation was issued as a result of the inspection and is listed on the violation notice.
