During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on October 9, 2025, and completed on October 22, 2025, in response to a complaint received by the licensing office on October 6, 2025, relating to allegations of Staffing and supervision and Programs. The inspector reviewed 1 child record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Physical plant, Special Services and Background Checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 29, 2025. A POC submitted after this date will not appear on the public website.
Violations
3Staff #1, who had been employed for four weeks, did not have a central registry search requested.
The center shall ensure that areas and equipment of the center, inside, are maintained in a clean, safe and operable condition.
There was peeling paint coming off of the wall in Classroom #2 and the couch in the classroom had a large tear in it exposing the foam inside. In classroom #1 and Classroom #2 pieces of yarn approximately 33 inches in length were hanging from the wall and accessible to children.
The children in Classroom #1(age: 2 years) and Classroom #2 (age: 2 years) were served large pieces of fresh pineapple for lunch.
