The center is required to have employees undergo a background check every five years.

The most recent fingerprint-based background check for Staff #2 was overdue by one week.
Plan of Correction: Staff completed the fingerprint on 1/31 and results were returned 2/3/26.

Prior to employment, the center is required to request background checks from any state in which the individual has resided in the preceding five years.

The center requested a search of the sex offender registry from one state for Staff #3 over eight weeks after their date of employment.
Plan of Correction: The record came back clear and we have updated our process to do that check prior to the first day of work.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #3 TB was dated six weeks prior to date of employment.
Plan of Correction: employee has since done a new TB screening that has come back clear.

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1 updated TB screening was due 3 weeks ago. The updated screening has not yet been completed.
Plan of Correction: Due to the weather, the staff members appointment was canceled. The have since completed the screening.

Hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

A bottle of cleaning spray was observed within reach of children, on top of a paper towel dispenser in the bathroom of Classroom #3.
Plan of Correction: The bottle of Fabreeze air effects was removed from the school aged bathroom and stored in a locked cabinet.

There shall be at least twelve inches of space between the sides and ends of occupied cribs.

In Classroom #1, the sides of the cribs for Child #1 and Child #2 were both occupied and positioned right next to each other. There was no space between the occupied cribs.
Plan of Correction: Immediately the cribs were rearranged, and staff were retrained on standards.
Standard 8VAC20-780-450-A
Cribs when being used by infants shall have a bottom cover.

In Classroom #1, a blanket was hanging on the inside of Child #2 (age: 8 months) crib. The blanket was clipped at the top rail of the crib and loosely tucked into the side of the mattress.
Plan of Correction: Staff immediately removed the blanket and were assigned additional training on safe sleep for infants.

The diapering area shall have a nonabsorbent surface for diapering or changing.

The diaper changing pad on the changing table in Classroom #2 had three small tears making it an absorbent surface.
Plan of Correction: A new changing mat was purchased, in addition to two back ups for the future. While waiting for delivery, we moved changes to another classroom.
