Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 11/08/2021 and concluded on 11/10/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 62 children present, ranging in ages from 6 months to 5 years ,with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
2Evidence:
1) The record for Child #2 did not contain a physical examination. There was a CoVA School Entrance Health Form in the record however, it was not completed nor signed by a physician.
Evidence:
1) The record for Staff #3 did not contain written information to demonstrate that the staff person possesses the orientation training required by the job position.
2) The center director stated that Staff #3 completed the orientation training but they could not locate the documentation in the record.
