Inspector Notes

An unannounced monitoring inspection was conducted on 04/05/2022 between the hours of approximately 12:20 pm through 2:00pm. There were 47 children present with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-245-J-3
Based on medication review and interview with staff, children for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed were not always in the care of a staff member who meets the requirements to administer the medication.
Evidence:
1) On the date of inspection, there was emergency medication in the center for Child #2. 2) Staff #1 stated that staff assigned to Child #2's class have not completed the Medication Administration Training (MAT) certification. Child#2's medication has been administered by Staff #1 who is MAT certified.
Plan of Correction: On 04/08/2022, we contacted 2 MAT trainers and requested a class to enroll the staff member in. Once the class has been scheduled, we will notify the inspector of the date of class.
Standard 8VAC20-780-330-B
Based on observation, resilient surfacing in the playground did not comply with minimum safety standards.
Evidence:
1) On the date of inspection, there was not enough resilient surfacing remaining in some areas on the playground. The level of the mulch was low in some areas and in others, dirt and ground was able to be seen from where the resilient surfacing has been kicked out, especially on the landing zones of the slides.
Plan of Correction: The mulch will be replenished within 30 days from inspection date.

Based on observation, food brought from home was not dated as required.
Evidence:
1) On the date of inspection, there were three bottles with milk in the infants room that did not have a date on them as required.
Plan of Correction: Bottles were labeled. Parents and staff were reminded about requirements.
Standard 8VAC20-780-70
Based on record review, staff records did not contain all required documentation.
Evidence:
1) The record for Staff#3 did not contain the address of a designated person to be notified in an emergency.
Plan of Correction: The staff member added an address for her emergency contact.
