Inspector Notes

A monitoring inspection was conducted on 11/15/2023 between the hours of approximately 11:15 am and 12:45 pm. There were 103 children present, with 8 staff supervising. Ratios were in compliance. Children in different rooms were observed while working with manipulatives, completing a fall art craft, listening to a story and eating lunch.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medication on site today. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not ensure to obtain fingerprint-based criminal history check determination letter every five years for employees.
Evidence:
1) On the date of inspection, the fingerprint-based criminal history check determination letter in the record for Staff #2 was dated 11/14/2018.
Plan of Correction: Corrected. All staff fingerprints completed and put in files.

Based on record review, the center did not ensure to obtain a completed Sworn Statement every five years for employees.
Evidence:
1) On the date of inspection, the Sworn Statement in the record for Staff #2 was dated 11/10/18 and for Staff #3 was dated 11/14/2018.
Plan of Correction: Corrected. New Sworn Statements were completed for each Staff #2 and Staff #3.
