During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/09/2025, as part of the licensure period.

The on-site inspection began at 11:10 and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above.

There were 44 children present and 11 staff. The inspector reviewed 4 children?s records and 5 staff records on-site.
This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 12/19/2025. A POC submitted after this date will not appear on the public website.

The findings from the most recent inspection, dated 04/08/2025, were not posted in a conspicuous place on the premises.

*REPEAT VIOLATION*
Orientation training shall be appropriate to the age of the children in care and include all of the required facility specific topics.
The orientation training for Staff #4, who has been employed for eighteen months, did not document having completed training in the
following topics: Prevention of sudden infant death syndrome and use of safe sleep practices; and prevention of shaken baby
syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children.
Plan of Correction: The missing orientation documents were reviewed, explained then signed by Staff #4.

Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing with the center's information listed in 8VAC20-780-420 A.
There was no signed acknowledgement of the Staff #4 having received the required information.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
A current fire inspection was not available. The most recent inspection report was dated 10/28/2024.
Plan of Correction: Fire Marshall came and completed our inspection on 12/18/2025.
