Inspector Notes

A renewal inspection was conducted on 5/09/2023 with the center director and staff. There were 29 children present, ranging in ages from 5 months to 6 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The children were having lunch and preparing for rest.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that areas of the center, inside and outside, shall be maintained in a clean and safe condition.
1. Numerous exposed tree roots were observed on the far preschool playground located in the corner of the play area.
2. Trash had accumulated on the ground, under the stairwell leading up to the school age building, accessible to children.
Plan of Correction: The area with tree roots will be supervised by staff.
The trash will be removed from under the stairwell.
Standard 8VAC20-780-430-L
Based on observation, the facility failed to prevent the use of infant walkers. Evidence: on May 9, 2023, there was a walker in the infant room and a walker on the infant outdoor play area.
Plan of Correction: The infant walkers were removed from the classroom and outdoor play area. They will not be used.
Standard 8VAC20-780-510-P
Based on review of 14 medications, the facility failed to notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Evidence: The medication authorizations for Child B expired 4/28/2023. The authorizations had not been renewed and the medications were not returned or destroyed as required. The medication was still on site.
Plan of Correction: The medication authorizations will be renewed or the medications will be returned to the parent.
