Inspector Notes

An unannounced non-mandated monitoring inspection was conducted today between the hours of 9:30am-11:30am. There were a total of 80 children with 14 staff members present. Children were observed during circle time, engaged in group activities, hand washing, and free play. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Based on record review and interview, the center did not conduct an out of state criminal history name check prior to the first day of employment.
Evidence:
1. Staff #2 (Date of hire-01/19/2022) lived in the state of Illinois in the past five years according to a signed Sworn Statement Disclosure dated 01/19/2022. Staff #2's record did not have documentation of an out of state criminal history name check for the state of Illinois.
2. Staff #3 confirmed that an out of state child abuse and neglect search request and sex offender registry name check was conducted, but no criminal background name check for the state of Illinois.
Plan of Correction: The center will conduct an out of state background name check for the state of Illinois and send the results to the Licensing Inspector.
