Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:30am-12:45 pm. There were a total of 92 children with 12 staff members present. Children were observed during water play, handwashing, diapering, toy play, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
10Evidence:
1. Child #5's record (Start date-1-20-22) did not contain additional immunizations once every six months. The last immunization record was dated 12/16/21.
Evidence:
In the three's classroom, five bottles of cleaner and sanitizer was located in a closet that was not locked.
Evidence:
1. In the Infant B classroom, cereal and snacks were stored on a bottom shelf with four bottles of cleaner and sanitizer.

1. In the laundry room, a bottle of bleach, powdered laundry detergent, cleaning materials, and paint were stored together.

1. One of the three toilets located between the 2's classroom and the Toddler classroom did not flush.

1. Child #2's record (Start date-1/4/21) did not contain documentation of an annual update,
Evidence:
1. In the toddler room, a child's hands were not washed after diapering.
2. Staff #3 did not wash their hands with soap and water after diapering.
Evidence:
1. In the Toddler room, the diapering table was not cleaned and sanitized after each use.
Evidence:
1. In the Infant B classroom, 1 of 5 diaper creams were not labeled and did not have parent authorization.
2. In the two's classroom, 8 diaper creams did not have parent authorization and 5 diaper creams were not labeled with the child's name.
3. In the Toddler classroom, 1 of 8 diaper creams were not labeled.
Evidence:
1. In the Infant B classroom, 6 of 6 accident reports were missing the actual time that parents were notified of the accident.
