Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 9:30am-11:45 am. There were a total of 69 children with 15 supervising staff members present. Children were observed during circle time, diapering, handwashing, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard 22.1-289-036-B-4
Based on record review, the child day center did not obtain a copy of the results of an out of state child abuse and neglect registry for an agent that has lived outside the state in the preceding years.
Evidence:
1. Agent #1 has lived in the state of Maryland in the preceding five years. A copy of the results an out of state Child Abuse and Neglect search was not available at the time of the inspection.
2. An out of state Child Abuse and Neglect search was sent out for Agent #1 on December 6, 2022.
Plan of Correction: The Out of State Child Abuse and Neglect Search was sent on December 6, 2022. Management will send the LI the results when they return.
Standard 8VAC20-780-240-I
Based on record review, documentation of orientation training did not include the required information.
Evidence:
1. Staff #5's orientation (Date of hire-08/18/2022) did not include the training delivery method.
Plan of Correction: Management will use the model form staff orientation for staff moving forward.
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Preschool Classroom, the couch located in the library area had two four inch rips in the cushion.
2. In the Discovery Preschool Classroom, the single chair located in the library area had a quarter size tear on the left and right arm of the chair.
3. In the Discovery Preschool Classroom, there was a dime sized hole in the wall containing a stud where a bookcase used to be located. The hole had sharp edges that could snag skin and entangle clothing.
Plan of Correction: The couch and the chair were removed from the classrooms during the time of the inspection. A couch from another classroom was put into the preschool classroom. The hole in the wall will temporarily be fixed by placing tape over the hole, but a work order will be placed to remove the stud from the wall and painted.
Standard 8VAC20-780-500-B
Based on observation, the diapering area did not have an appropriate disposal container.
Evidence:
1. In the Discovery Preschool classroom bathroom, a foot operated trashcan did not have a plastic liner located inside. A pull up was observed in the trashcan.
Plan of Correction: A plastic liner was put into the trashcan in the bathroom and the pull up was removed.
Standard 8VAC20-780-540-C
Based on observation, first aid kits did not contain required items.
Evidence:
1. The first aid kit did not two triangular bandages.
Plan of Correction: Multiple Triangular bandages were purchased for the center at the time of the inspection.
