During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on May 6, 2025 and completed on May 6, 2025. The on-site inspection began at 10:45am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 64 children present and 11 staff. The inspector reviewed 7 children?s records and 5 staff records [on-site].
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 15, 2025. A POC submitted after this date will not appear on the public website.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standard and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Slides and a climbing structure on the outdoor playground were observed without the minimum depth of resilient surfacing in the fall zone.
Plan of Correction: A machine has been rented to move the pea gravel away from the fence and place it under the playground equipment. This will take place 5/12/25 and 5/13/25.
