Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 12:00pm-2:00pm. There were a total of 31 children with seven staff members present. Children were observed during lunch time, diapering, handwashing, and nap time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
9Evidence:
1. Staff #3's (Date of rehire-12/13/21) record did not have documentation of a five year repeat Central Registry results. The Central Registry results was dated 04/21/2017.
Evidence:
1. Staff #1 (Date of hire-3/23/22) did not have documentation of fingerprint results in the record.
2. Staff #3 (Date of hire-12/13/21) did not have documentation of fingerprint results in the record.
Evidence:
1. Child #1 (Start date-5/20/19) did not have documentation of a physical examination.

1. In the Toddler classroom, the changing pad had three one inch tears on the surface.

1. The evacuation drill log did not have documentation of a monthly evacuation drill for the months of January, March, and April.
2. Staff #4 confirmed that a practice evacuation drill was not conducted for the months of January, March, and April.
Evidence:
1.In the Toddler classroom, a child was allowed to walk around while drinking a bottle.
Evidence:
1. In the Infant 2 classroom, two older infants were seated at a table eating while unsupervised. One older infant was seated in a highchair while eating unsupervised.
Evidence:
1. Two bottles located in the refrigerator in the Infant 2 room were not dated.
Evidence:
1. Child #2's record did not contain a second emergency contact.
2. Child #1's record and Child #3's record did not contain documentation of an annual update.
