An unannounced complaint investigation was conducted this date. Staff Interviews were conducted and records were reviewed in response to a complaint alleging forbidden action and care and protection in the Bridge classroom. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid. Violations were issued not related to the complaint. Based on record review and staff interview, the center did not conduct a fingerprint based criminal history check prior to the first day of employment.
Evidence:
1. Staff #1(Date of hire-04/29/22) did not have documentation of fingerprint results in the record.
2. Staff #2 (Date of hire-04/19/22) did not have documentation of fingerprint results in the record.
3. Staff #3 (Date of hir-04/21/22) did not have documentation of fingerprint results in the record.
