Inspector Notes
An unannounced renewal inspection was conducted today between the hours of 11:00am-1:30 pm. There were a total of 40 children with 11 supervising staff members present. Children were observed during lunch time, handwashing, diapering, engaged in group activities and outdoor play time. A selection of staff and children records and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
7Evidence:
1. Staff #1's record (Date of hire-03/13/2023) contained documentation of fingerprint results after the first day of employment. The fingerprint results were dated 03/16/2023. The record contained documentation of a Sworn Statement dated 03/13/2023 and staff orientation dated 03/13/2023.
2. Staff #1 confirmed that the fingerprint results were obtained after the first day of employment.
Evidence:
1. Staff #1's record (Date of hire-03/13/2023) contained a Sworn Statement that states that they live in Washington D.C. Staff #1's record did not contain documentation of an out of state criminal background check for Washington D.C.
2. Staff #1 confirmed that a copy of the results of a criminal background check for Washington D.C. had not been obtained prior to the first day of employment. Staff #1 stated that an out of state criminal background check has been submitted for Washington D.C., but results have not been returned.
Evidence:
1. Child #1's record (Start date-01/17/2023) did not contain documentation of a physical examination.
2. Child #5's record (Start date-03/13/2023) did not contain documentation of a physical examination.
Evidence:
1. Staff #1's record (Date of hire-03/13/2023) contained documentation of a negative TB screening at the time of employment. The TB screening was dated 03/16/2023.
2. Staff #3's record (Date of hire-04/21/2023) contained documentation of a negative TB screening not within the last 30 calendar days of the date of employment. The TB screening was dated 02/07/2023.
3. Staff #4's record (Date of hire-04/11/2023) contained documentation of a negative TB screening not within the last 30 calendar days of the date of employment. The TB screening was dated 02/01/2023.
Evidence:
1. In the Toddler classroom, a bottle labeled bleach and water and soap and water were sitting on a high shelf and not kept in in a locked place using a safe locking method.
Evidence:
1. In the Infant classroom, a container of bleach sanitizing wipes was located in a cabinet with containers of applesauce, infant formula, plates, bowls, and spoons.
Evidence:
1. In the two year old classroom, Staff #6 (Date of hire 08/17/2022) did not wash their hands before or after a diaper change.
