During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on June 9, 2025 and completed on June 9,2025 in response to a complaint received by the licensing office on June 2, 2025 relating to allegations of abuse. The inspector reviewed 1 child file and 2 staff records. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
Though not a part of the original complaint, violations that were not reported were found related to emergency preparedness and care, are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/14/2025. A POC submitted after this date will not appear on the public website.
Violations
2When staff are supervising children, they shall always ensure their care, protection, and guidance.
Child A was running on the basketball court on the local playground, when they tripped and fell, scraping their forehead over and under the left eye. Staff 1 stated that the child got up and continued to play, so no first aid was given. Staff also stated that abrasions were noted after the child woke up from naptime, and no first aid was given at that time.
All staff members have been advised to administer first aid immediately when a child injures themselves.
Additionally, incident reports will be completed as soon as possible, but no later than 30 minutes after the incident occurs.
Staff 1 confirmed that there was not an ice pack in the 1st aid bag at the playground with the children at the time that Child A was injured. Staff 1 stated that the ice pack had been used and not replaced yet.
All staff members have been advised to inspect their first aid and emergency supplies daily to restock missing items.
