During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced inspection was initiated on July 1, 2025 and completed on July 1, 2025 in response to a complaint received by the licensing office on June 25, 2025 relating to allegations of ratio, care and supervision of children. The inspector reviewed 3 child files and 2 staff files. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 7. A POC submitted after this date will not appear on the public website.

Each child shall have a physical examination by or under the direction of a physician:
Before the child's attendance; or within 30 days after the first day of attendance.

Child B and C, both in attendance for over 6 months, have no record of a physical available for review.
Plan of Correction: The registrar (principal) will ensure that enrollment packets have a physical examination by or under the direction of a physician: 1. Before the child's attendance; or 2. Within 30 days of the first day of attendance.

Contacted parents of B & C. The parents are aware that a physical examination is needed as soon as possible. They have contacted their child's physicians and will submit a comprehensive physical examination report.
