SYSTEMIC DEFICIENCY
No child day center shall hire for compensated employment or continue to employ any person who has been convicted of a barrier crime.

The national criminal background check (applicant screening request) completed for Staff #3 was returned with a result that the individual was not eligible as employee. Staff #3?s employment was not immediately terminated when the not eligible result was received 26 days after the first day of employment. Staff #3 was employed by the Center for six months with the ineligible background.
Plan of Correction: Effective immediately, Central Office will designate potential hires as temporary appointment pending review (TAPR) at our preschool academy once the following are received: criminal background check results, a completed sworn disclosure statement, FBI background check results, local background checks, proof of required central registry check (including any state of residence in the last 6 years), negative TB screening results, and at least 2 references to character, reputation, and competency.

Packet will be provided to the preschool principal for review after completion for verification prior to the individual being designated as a new hire.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment.

Staff #3 who had been in contact with children, had documentation of TB screening dated four days after date of hire.
Plan of Correction: Effective immediately, Central Office will designate potential hires as TAPR at our preschool academy once the completion and receipt of negative TB screening results that are not more than 30 days old.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On February 10, 2026, Staff #1 roughly handled and threw a blanket at Child A in a manner that did not ensure their care, protection, and guidance.
Plan of Correction: The preschool academy principal convened a meeting of all staff on February 10, 2026, at which he reviewed the preschool academy's behavioral guidance policy and practices.

The Family Support Manager will provide behavioral guidance training and conduct regular classroom observation to monitor implementation of appropriate behavioral guidance procedures.

On February 10, 2026, during naptime, Staff #1 physically moved Child A by the back of their pants and their arm, when Child A was off their cot and near another sleeping child in the classroom. The distance Child A was moved was approximately 15 feet.
Plan of Correction: The preschool academy principal convened a meeting of all staff on February 10, 2026, at which he reviewed the preschool academy's behavioral guidance policy and practices.

The Family Support Manager will provide behavioral guidance training and conduct regular classroom observation to monitor implementation of appropriate behavioral guidance procedures.

The following action is forbidden: roughly handling a child.

On February 10, 2026, during naptime, Staff #1 was observed picking up Child A by the
back of their pants and then their right arm to move Child A to the other side of the classroom, approximately 15 feet. Staff #1 then threw Child A's blanket at them, after they were on their cot.
Plan of Correction: The preschool academy principal convened a meeting of all staff on February 10, 2026, at which he reminded those present that Hopkins House maintains a zero tolerance policy for child abuse and neglect, and that they share accountability in maintaining a safe environment and are expected to strictly adhere to the preschool academy's "see something, say something" approach to child safety. The academy principal will ensure that ongoing training regarding child abuse, mandated reporter, and other relevant policies and practices are conducted and documents throughout the preschool year. He will provide coaching and conduct regular classroom observation to monitor implementation of appropriate child safety procedures.

Up-to-date information is required to be requested from the parent for each child's record at least annually.

The information in the record of Child D had not been updated in over 15 months which is overdue by three months.
Plan of Correction: The academy assistant principal has reviewed all records maintained in the academy's records management system (Brightwheel) and worked with the parents to update their child's records that have not been updated within the past 12 months.
