During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/24/26. The on-site inspection began at 10:15am and ended at 12:40pm The inspector reviewed compliance in the areas listed above. There were 34 children present and 6 staff. The inspector reviewed 6 children?s records and 5 staff records on-site on 3/24/26. Additional records were viewed electronically on 3/25/26. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/13/26. A POC submitted after this date will not appear on the public website.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 had no documentation and had been working in the classroom for one month.

Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record, and shall include all the required information. The record for Staff 1 did not contain documentation of orientation training.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff # 2 changed a child?s diaper in the two year old room and did not wash the child's hands with soap and water.
