Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and interviews.

A complaint inspection was initiated on 6/7/2021 and concluded on 8/2/2021. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, staffing and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The information gathered during the the investigation supports the allegations, so the complaint is found to be valid.

Standard 22VAC40-185-340-A
Based on interviews the licensee failed to ensure the care, protection and guidance of a child in care.
Evidence:
1. Staff #1 and #3 confirmed that on June 7, 2021 on or about 5:00 p.m. child #1 was erroneously dismissed from his classroom for parent pick up.
2. Child #1 walked unsupervised down the hallway and exited through the facility?s main door.
3. Staff #3 confirmed that there was not a staff person present at the front desk or entrance to the facility at the time.
4. At approximately 5:15 p.m. a witness saw child #1 standing unsupervised in the parking lot ?about 10 feet from the first parking spot?.
5. Two witnesses stated that while child #1 was standing in the parking lot, there were multiple families picking up children from care and moving cars entering and leaving the parking lot.
6. Child #1?s father arrived to pick up child #1 and discovered him standing unsupervised in the parking lot.
Plan of Correction: The following plan of correction was submitted by the facility:
"A staff member is always at the front door".

Based on interviews, the licensee failed to ensure that children under the age of 10 years are in sight and sound supervision of staff.

Evidence:
1. Staff #1 and #3 confirmed that on June 7, 2021, a three-year-old child (child #1) was erroneously dismissed from his classroom and was out of sight and sound supervision of staff. The child was unsupervised for a minimum of 7 to 8 minutes.
2. Child #1?s father discovered child #1 standing unsupervised in the parking lot when he arrived to pick him up at approximately 5:10 p.m.
Plan of Correction: The following plan of correction was submitted by the facility:
"Parents are allowed in the building to receive their child directly from their class".
