During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 6/9/2025. The on-site inspection began at 10:20 am and ended at 1:05 pm. The inspector reviewed compliance in the areas listed above. There were 99 children present and 17 staff. The inspector reviewed 9 children?s records and 9 staff records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/18/2025. A POC submitted after this date will not appear on the public website.

(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state for Staff #8, who has been employed for more than ten months.

Standard 8VAC20-780-160-A
(Repeat Violation) Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #3 TB was dated 4 days after the date of employment. Staff #4 TB was dated a month after the date of employment. Staff #7 TB was dated six months prior to date of employment.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There were disinfectant wipes on shelves and on counters, not within reach of children.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information. The record for child #8, enrolled for two months, was missing one parents work phone number and place of employment and documentation of a physical. The record for child #9 was missing the address for one of the emergency contacts.
