A subsidy inspection was conducted on 07/14/2022. Staff and children's files were reviewed, interviews conducted with staff in addition to a tour of the facility. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a record review the vendor did not ensure all staff had subsequent tuberculosis screenings at least every 2 years since the last screening.

Plan of Correction: The staff had a chest x-ray done and thought it was good for years, per physicians instructions. A TB screening will be obtained this week.
