A subsidy inspection was conducted on 07/17/2023 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor did not have a written allergy care plan for Child#2 who had a documented diagnosed food allergy.
Plan of Correction: The director will contact the parent to obtain documentation of a written allergy plan for child #1

Based on observation the vendor did not ensure hazardous substances were kept in a locked place.

There were cleaning agents on a table in the multipurpose room and there was an air air freshener containing a liquid chemical plugged into the wall in the bathroom.
