In order to promote the child's physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, support.
Documentation reviewed and interviews conducted on 11/06/25 and 11/13/25 revealed that Staff #1 was observed yelling at staff and slamming classroom doors on several occasions in front of the children in care.

Plan of Correction: Per the Center:
"The staff member involved was removed from child contact immediately and is no longer employed by the company. Leadership reviewed documentation and conducted interviews to address professional conduct expectations.
Leadership reinforced expectations regarding professional communication, emotional regulation, and appropriate conduct in the presence of children.
Classroom environments are monitored as part of routine center leadership oversight to ensure emotional safety and appropriate staff behavior."

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On 10/29/25, Staff #1 said to Child #1 (6-year-old) that they were going to stab them in the back of their head if they approached another parent.

Plan of Correction: Per the Center: "Immediately upon learning about the incident, the staff member involved was removed from all child contact and is no longer employed by the company. Leadership conducted an internal review to assess compliance with behavioral guidance requirements.
Corrective Actions Implemented:
On 1/22/26, all staff completed refresher training on positive behavior guidance, age-appropriate redirection, and respectful communication, aligned with company policy and state licensing requirements.
Behavioral guidance expectations and zero-tolerance standards for inappropriate or threatening language were reviewed with staff on 1/22/26.
Leadership reinforced observation and documentation practices related to staff-child interactions, with coaching provided as needed."
