The required ratio for children birth up to 16 months is one staff per four children.
Documentation reviewed and interviews conducted on 11/06/25 and 11/13/25 revealed that on 10/29/25, Staff #1 and Staff #2 were alone with 11 children, ages ranging from 4 months up to 14 months, for over an hour. On 10/31/25, Staff #1 and Staff #2, were alone with 11 children, ages ranging from 4 months up to 14 months, for approximately 10 minutes.

Documentation reviewed and interviews conducted on 11/06/25 and 11/13/25 revealed that on 10/29/25, Staff #1 and Staff #2 were alone with 11 children, ages ranging from 4 months up to 14 months, for over an hour. On 10/31/25, Staff #1 and Staff #2, were alone with 11 children, ages ranging from 4 months up to 14 months, for approximately 10 minutes.
Plan of Correction: Per the Center:
"Immediately upon learning about the staffing concern, classroom coverage was adjusted to restore compliance with required staff-to-child ratios. Leadership reviewed staffing practices and supervision procedures to ensure alignment with licensing requirements.
Corrective Actions Implemented: Staff were reminded of required staff-to-child ratios appropriate procedures for requesting additional support.
Leadership reinforced daily staffing oversight and real-time monitoring of classroom ratios."

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
On the morning of 10/29/25, Staff #1 was the only staff member present with 16 children, ages ranging from 3 years old to 11years old, in the Youth Zone classroom. The staff-to-child ratio based on the age of children in care was one staff to ten children. It is estimated Staff #1 was out of ratio for approximately 45 minutes.
The Youth Zone classroom is used as an opening classroom where Prep (three and four year olds), Junior Kindergarten (four and five year olds), and Youth Zone (school age) classrooms are regularly combined in the morning.

Bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location.
Interviews conducted on 11/06/25 and 11/13/25 revealed that on 10/31/25, Staff #3 was observed giving Child #1 (9-months-old) a bottle while the child was in their crib.
Bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location.
