Inspector Notes
An unannounced inspection was initiated on 01/23/26 and completed on 02/12/26, in response to a complaint received by the licensing office on 01/21/26 relating to allegations of administration; staff qualifications; programs; staffing & supervision; and special care provisions. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
