Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 1/05/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 80 children present, ranging in ages from infants old to 12 years old, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Based on record review, the center did not obtain a copy of the results of a sex offender registry check (prior to the 1st day of employment), criminal history record information check (prior to the 1st day of employment) or a search of the child abuse and neglect registry (search by the end of the 30th day of employment) from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #1 (Date of hire 11/05/2021) lived in Rhode Island (RI) and Tennessee (TN) in the last 5 years and the center did not have the results of the sex offender registry check for RI or TN (prior to the 1st day of employment). The center did not have the criminal history record information check for RI (prior to the 1st day of employment). The center did not have the search of the child abuse and neglect registry for RI (search by the end of the 30th day of employment).
Plan of Correction: My plan of correction is to send out the missing Background checks tomorrow, January 6th. And going forward I will be sure to complete all sex offender and background checks prior to day 1 of employment.
