Inspector Notes

An unannounced monitoring inspection was conducted on 2/22/2024 from 9:45 am-to 12:00 pm. There were 111 children supervised by 22 direct care staff. The physical plant, 10 children?s records, 5 staff records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in story time, playing outside, writing, tabletop and floor activities, play dough, and reading. Areas of non-compliance are identified in the Violation Notice. Based on record review and interview, it was determined the center did not ensure that the record for each child contained documentation of child updates and confirmation of up-to-date information in the child's record as required by 8VAC20-780-420 E 3.

Evidence:
1. Child #3's last annual update is 9/21/2022.
2. Child #5's last annual update is 9/26/2022.
3. Child #6's last annual update is 12/28/2021.
4. Child #7's last annual update is 9/21/2022.
5. Child #10's last annual update is 8/1/2022.
Plan of Correction: We will have the parents update their information by 2/22/2024.
We will have the parents use the new emergency release form as an annual update.
