During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 3/18/2026 and completed on 3/31/2026 in response to a self-report received by the licensing office on 3/6/2026, relating to supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/6/2026. A POC submitted after this date will not appear on the public website.
Violations
2On Friday, March 6, 2026, Child #1 (age 3) was left unsupervised in the bathroom for approximately five minutes during a group bathroom break. Staff #1 was unaware that the child had been left in the bathroom. Staff #1 reported that they did not take a headcount and did not have the CRS (Child Supervision Record) with them.
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
The center did not follow its own policies and procedures, as required by the standards for identifying where children are at all times.

The center?s procedures for identifying where children are at all times require staff to use a tool called the Child Supervision Record (CSR) used to track where children and teachers are throughout the day. A process called Name-to-Face is used to account for children, comparing each child?s name on the CSR to their face, confirming they are present.
