During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 4/29/2025, as a part of the provisional licensure period. The on-site inspection began at 11am and ended at 12:37pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 10 staff. The inspector reviewed 3 children?s records and 3 staff records.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/19/25. A POC submitted after this date will not appear on the public website.
Violations
6Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or equivalent, and meet one of the following.
Staff 1, 2, and 3 did not have documentation of program leader qualifications.
The Virginia Department of Education - sponsored orientation course shall be completed within 90 calendar days of employment.
Staff 1 and 2 did not complete the sponsored orientation course, and have been employed for more than 90 days.
1. Room 6 had peeling paint on the wall near the rear door.
2. Playground had a plastic bucket that was broken and had sharp edges.
3. Several nails sticking out of the fence boards, and fence posts, along the both playgrounds.

Room 1 had Febreeze, glass cleaner, sanitizer, and TB disinfectant accessible to children in an unlocked closet.
Room 2 had 1 uncovered electrical outlet by the children's handwashing sink, and 2 uncovered electrical outlets under the television.
Room 5 had formula for an child in care, but did not have a name on the formula, identifying the owner.
